This page is only for those Class Members who received an email notice letting them know that there was an issue with their claim submission, and that they needed to provide documentation in order to cure their claim. The deadline to file a claim passed on October 14, 2025.
If you received an email notice from the Claims Administrator containing a Unique ID and PIN informing you that there was an issue with your submitted claim, and that the Claims Administrator needed you to provide information or other documentation in order for your claim to be considered valid under the terms of the Settlement, please login below using the Unique ID and PIN included in the email you received. Submissions must be completed by the deadline on the email you received. You will also be asked to provide the date and city where at least one of your relevant purchases was made.
Please note that only purchases from a Whole Foods Market store located in California between November 3, 2017, and March 13, 2025 (the “Class Period), or, if purchased remotely (such as online), while you were present in California, are eligible for this Settlement. The relevant product includes the 365 brand Organic Hot Cocoa Rich Chocolate Flavor Mix.
You may also send copies of your documentation by mail to the address below. Please do not send original documents, as the Claims Administrator may not be able to return them. Please include your Unique ID and PIN in your mailing.
